Description: 
Mary Shofner, a dementia patient was living at Maywood, a KIndred facility temporarily following a respiratory condition. Ms. Shofner was severely neglected resulting in weight loss, immobility, unexplained bruises on her chest and neck, as well as bedsores down to the bone on her hip and buttocks that never healed and were allowed to become infected. She died 79 days after her admission to Maywood.
