Description: 
Mildred Smith was living at Little Rock Healthcare and Rehabilitation Center. During her stay she had to be transported to the dentist to get new dentures. Facility staff did not strao her into her wheelchair securely which caused her to fall out during the trip. She suffered from a broken ankle and bruising on her face, arm and leg. After the accident, she lost 13 pounds in less than one mo month and developed bedsores 15 days of the accident, three of which resulted in infections that required sharp debridement.
