Mamie Crook was an Alzheimer's patient living at Mariner's Clinton Health & Rehabilitation Center. While there she suffered multiple infected necrotic bedsores on her hip, ankle and heel, resulting in a left leg amputation. Facility staff also engaged in fraud by altering Crook's medical chart and facility staffing records.
