Annie Allen was sexually assaulted by a mentally handicapped male resident while both resided at the Shady Lawn Nursing Home. The facility did not screen new residents properly which led to the assailant being admitted as a resident. The nursing home neglected Allen and failed to provide her with a safe environment due to chronic understaffing.
